Preface

The Sixth Edition of The American Psychiatric Publishing Textbook of Psychiatry has undergone the most comprehensive and significant changes in its 25-year history. Although the book remains a one-volume, clinically oriented textbook, we have crafted this edition to be more practical and useful for busy clinicians and psychiatry residents in caring for patients and preparing for certification and recertification examinations.

The book is divided into four parts: Assessment and Development, Psychiatric Disorders, Treatments, and Special Populations. Part I includes chapters on the psychiatric interview and mental status examination, DSM-5, psychological assessment, laboratory testing and imaging, normal child and adolescent development, psychiatry and the law, and ethics in clinical psychiatry. Part II is organized in the exact same order as DSM-5 and covers all of the disorders contained within that volume. Part HI includes chapters addressing treatments—both psychopharmacological and nonpharmacological somatic and brain-stimulation treatments, and also the major forms of psychotherapy: brief psychotherapies, psychodynamic psychotherapy, mentalization psychotherapy, cognitive-behavioral therapy, and supportive psychotherapy. All of the psychotherapy chapter authors have corresponding American Psychiatric Publishing (APP) books, most of which feature video segments. The authors of most of the chapters in this Sixth Edition are psychiatrists and psychologists who have written textbooks or professional books for APP. In fact, 28 of the 38 chapters were written by APP book authors.

The Sixth Edition of the Textbook has 106 authors. Significantly, 66 of the authors are new to this edition, and in chapters that were written by the same authors in the previous edition, 42 of the coauthors are new. For the chapter authorship, we encouraged senior authors to include more junior colleagues to contribute and add freshness to the perspective conveyed in the chapters by their senior colleagues. We eliminated seven chapters from the prior edition and added several chapters that we believe are particularly relevant to clinicians. Chapter 2, titled "DSM-5 as a Framework for Psychiatric Diagnosis," provides the reader with a sound foundation on the preparation, rationale, and basis of DSM-5. Several new chapters were added based on the new DSM-5 criteria, with emphasis on explaining how these criteria have been reconceptualized in DSM-5. These new chapters are "Neurodevelopmental Disorders," "Obsessive-Compulsive and Related Disorders," "Trauma- and Stressor-Related Disorders," and "Elimination Disorders." In Part IH, Treatments, there is a marvelous new chapter titled "Mentalizing in Psychotherapy."

This edition is the first to incorporate the new DSM-5 criteria throughout, and 37 of the chapter authors were the researchers and intellectual leaders who participated in the DSM-5 process. The Textbook is a definitive source on DSM-5 and as such is intended to serve as a valuable reference for learners through experienced practitioners in the field. We were honored that David Kupfer, M.D., Chair of the DSM-5 Task Force, wrote the Foreword. He placed into perspective the many changes that were made in DSM-5 and how they will affect the delivery of psychiatric care to our patients.

The smaller size of the Sixth Edition of the Textbook is another development. We realize that clinicians, academicians, researchers, and trainees are quite busy taking care of patients, teaching, and conducting research. Consequently, we substantially reduced the length of the book (from more than 1,700 pages in the traditional larger textbook format of the Fifth Edition to just over 1,500 pages in the smaller, DSM-5-sized format of the Sixth Edition). This change, when the conversion to the smaller format is taken into account, represents about a 25% reduction in length over the Fifth Edition.

We wanted the Textbook to be the same size as DSM-5 to facilitate cross-referencing and so that the two volumes could be placed on the bookshelf next to each other. Thus, when a particular disorder is reviewed in DSM-5 and more information is required for patient care, the Textbook of Psychiatry can be readily accessed. The same would hold true for readers of the Textbook who want more information on the diagnostic criteria for a specific disorder.

We also asked the authors of each chapter to end their chapters with a brief list of key clinical points. Feedback from readers has shown this feature to be quite helpful. In addition to this feature, we requested that authors include recommended readings or websites that clinicians could utilize or recommend to their patients.

For the first time as a co-editor in the Sixth Edition of the Textbook we are pleased to include Laura Weiss Roberts, M.D, M.A. Dr. Roberts is the Katherine Dexter McCormick and Stanley McCormick Memorial Professor and Chair of the Department of Psychiatry at Stanford University School of Medicine. Dr. Roberts is also the Editor-in-Chief of Academic Psychiatry and has written extensively on ethics in psychiatry.

We have been in contact with many of the professionals who purchased a previous edition of the Textbook and have benefited from their comments and suggestions. Work on the Sixth Edition began in early 2011. Consequently, this edition represents the culmination of a 2-year effort to provide the most useful and up-to-date clinical information in the field.

In addition to being available to readers in either hardcover or e-book format, the Textbook of Psychiatry is included on PsychiatryOnline (psychiatryonline.org). We encourage readers to either purchase PsychiatryOnline themselves or have their institution do so. This electronic portal enables readers to access the information contained in the Textbook wherever they are.

All chapters were reviewed by a member of our distinguished Editorial Board as well as by the three of us. You will note that the Editorial Board represents a wide range of clinical, research, and academic interests and comprises senior psychiatrists who are well respected in their fields.

Similar to the previous edition, we have included a study guide—this time authored by Philip Muskin, M.D. The study guide to the Textbook is also available as a guide on PsychiatryOnline, and it includes more than 500 questions and annotated answers corresponding to the Textbook chapter pages. The online version of the study guide offers 20 American Medical Association Physician's Recognition Award Category 1 credits and has been approved by the American Board of Psychiatry and Neurology as part of a comprehensive lifelong learning program, which is mandated by the American Board of Medical Specialties as a necessary component of Maintenance of Certification.
